- A medical
history is required of patients being admitted from home.
- Pre-admission
assessment by our nurse liaison.7-day-a-week admission policy.2-hour
response time for referrals .Communication and coordination with
hospital social workers to ensure a smooth transfer .A tour of
our facility will be provided as well as detailed information
about services we offer.
- Questions
regarding finances will be answered in advance.

Multi-disciplinary team will create an individual plan of care based
on the physical, emotional, and spiritual needs of each resident
at the time of admission. The goal of each care plan is to maximize
appropriate clinical and financial resources incorporating care
needs. Our ultimate goal is for our residents to return home to
their regular daily activities at the earliest possible date. |
Discharge
Process:
Discharge
planning is a continuum in each residents plan of care. The facility
will coordinate discharge plans with other insurances to ensure
residents are exposed to the resources they need. |
